The gameplay in Cream Wolf consists of 3 different parts. The first part consists of the player driving around the neighborhood during the day in an ice cream van while playing music to attract little kids, collecting ice cream cones, and avoiding anyone else in vehicles that are suspicious of your motives. Touch a police car, fire truck, person in a car or TV van and you will lose a life. Collect 50 cones and you will get a speed boost.

When kids are close enough, they will want to get ice cream from you, which will start the ice cream serving process. Kids order stacks of ice cream scoops and toppings. Ice cream is served by pressing the button to pick ice cream scoops and toppings off a constantly rotating wheel. Based on your timing, the ice cream and toppings can be better aligned upon the cone which will earn you more points. Add an incorrect item and the kid will leave.

The last part of the game arrives at the end of the last day. It is night time and time to lure all the children you can back to your house. Paradoxically, kids you've fattened up more over the past few days will run faster to keep up with your van and will hear your music from further away. Other cars are still dangerous so it can be tricky to get all the kids back to your house without getting caught. After you get back to your house, you transform into a werewolf and have to eat the horrified, panicking children while avoiding any incoming police cars. Doing so will earn you more points and ice cream flavors such as Brain Freeze, Chocolate Hip, Knee-opolitan, and Kidney Stone Road which are clearly made from the children you've eaten.

Each additional town you visit will have more cars out to get you, but also more children for you to fatten and eat.
